I agree and authorize the Asia Hapkido Federation to collect personal data as stated in the "Personal Data Collection Statement." The personal data collected by the Federation from the applicant is provided voluntarily by the applicant, and it is ensured that such data will only be used for the specified purposes. Personal data may be used for processing all applications for the Federation's professional qualification certificates, identifying the identities of competition participants, disseminating news and information, or for internal reference and statistical data. The Federation will not use the personal data provided by the applicant for any other purposes without notifying or obtaining the applicant's consent.

IMPORTANT NOTICE :

1) To apply for the 1st Dan, one (1) photo and one (1) copy of the ID Card must be submitted together with this application form. (Add the word "COPY" across the copy of the ID card.)

2) To apply for the 2nd Dan and above, one (1) photo and one (1) copy of the current Dan Certificate must be submitted together with this application form. Regardless of whether it is small or large.

3) In the case of any false information provided in this application form, the Dan certificates issued under this application form shall be void, and the parties involved shall be subject to disciplinary action.
